It is important to note that the membership model will be in place for primary care patients only. Naturopathic patients will continue to see their providers without any additional cost.

Membership FAQ – Primary Care at CNHP

The membership model applies only to patients receiving primary care services. If you see one of our naturopathic doctors exclusively, nothing will change for you.

There are many behind-the-scenes costs that come with running a truly patient-centered practice. One of the most important is making sure we hold time every day for patients who need same-day care. With our membership model, we are expanding this commitment so that urgent care visits are rarely needed.

Our on-call service is always covered by one of our own providers—not a large call center or hospital nurse. They have access to your chart and are available 24/7 for emergencies. We want your team, who knows you, taking care of you.

We also never double-book appointments. Our goal is to maximize your time with your provider and ensure you always feel cared for and supported.

Family medicine is the core of our medicine. We honor the importance of supporting the whole family and offer a discount to families who are all invested in primary care in our office. To be considered for a family membership, the family should:

  • share the same primary residence
  • be on the same insurance policy, exceptions for disability
  • members will all be over age 14, as pediatric memberships are separate. 

Yes, this hybrid concierge model allows us to offset our overhead with a membership fee but still allows us to stay as affordable as possible by billing insurance for office visits.

Yes. The membership is a monthly fee that supports services not covered by insurance. This fee is in addition to any insurance-billed visits and covers all the services and care that insurance does not.

Simply put, because you deserve better care. Insurance models often force providers to see patients for short and treat symptoms with prescriptions. That’s not how we practice medicine. A membership model gives us the time and flexibility to provide the thoughtful, whole-person care you expect from us—focused on root causes and long-term wellness.

We presented our pain points and stressors as a practice to our patient advisory board last summer. We heard their feedback as patients and their experiences. We researched various solutions and had a strong endorsement from the whole board for a membership program to enable us to continue to practice in line with our values and still accept insurance and be as accessible as possible.

Yes. We have a hardship and financial assistance option available. If cost is a concern, please don’t hesitate to reach out—we’ll work with you.

Yes. Patients of the primary care team, as well as those on the waitlist prior to January 1, 2026, will be offered a transition discount. This is our way of honoring long-standing patients and supporting them as we move into this new model of care.
