This information is critical to getting your office visits paid for. If we do not have this information, you will be responsible for the entire cost of your visit. If you have new insurance and would like to verify insurance coverage, please call our office and ask for our billing team.

3. Please note, we are licensed practitioners in Connecticut. For all insurance based visits, patients MUST be in Connecticut at the time of the visit. Any appointments outside of Connecticut will be cash based health coaching only.
